  • Asli Kutipan Akta Nikah / Duplikat Akta Nikah;
  • Foto copy Kutipan Akta Nikah / Duplikat Akta Nikah, 1 lembar kertas A4;
  • Foto copy KTP Penggugat/Pemohon, 1 lembar kertas A4;
  • Foto copy Surat Ijin Atasan (bagi PNS / TNI / POLRI), 1 lembar kertas A4;
  • Untuk suami / istri yang tidak jelas alamatnya (Ghaib), melampirkan pula Asli dan Foto copy Surar Keterangan Ghoib dari Desa/Kelurahan setempat yang menyatakan bahwa suami / istri telah pergi meninggalkan rumah sejak bulan …….. tahun …….. sampai sekarang dan tidak diketahui alamat jelasnya, 1 lembar kertas A4;
  • Membayar biaya panjar perkara sesuai SK Panjar yang berlaku.

  • Foto copy Kutipan Akta Nikah/duplikat Kutipan Akta Nikah Pewaris, 1 lembar kertas A4;
  • Foto copy Kartu Keluarga Pemohon/Para Pemohon, 1 lembar kertas A4;
  • Foto copy akta kelahiran/akta kenal lahir, Pemohon/Para Pemohon, 1 lembar kertas A4;
